Delhi’s Medical Navratnas announced

Shri Narender Modi, Hon’ble Prime Minister of India recently announced the Navratnas of the country and asked to follow the “Chain of 9” in every city. In a function held few days back, I was nominated as one of the Navratnas of Delhi for the purpose.

To complete the “Chain of 9”, I was entrusted with the job of motivating and nominating 9 medical icons “Medical Navratnas” in the city of Delhi. I have nominated the following nine doctors as Medical Navratnas:

This will be a historic moment for the city of Delhi where top medical icons will participate and spread the message of Swachh Bharat Swastha Bharat. All 9 of them would also participate in a symbolic cleanliness drive in the indoor stadium.

Safety of dronedarone for rhythm control of atrial fibrillation

Dronedarone is a class III antiarrhythmic drug. There has been concern about the safety of dronedarone, especially for patients with heart failure and permanent atrial fibrillation (AF). An analysis of 174,995 patients with a diagnosis of AF during 2010 to 2012 published in J Am Coll Cardiol. 2014 Jun 10;63(22):2376–84 concluded that dronedarone did not increase risks of death or liver disease.

  • Roche Holding AG has filed its Ebola virus test for emergency use approval in the United States, in response to the world's worst outbreak of the disease. The company has also filed for the CE mark approval in the European Union. The test can run on polymerase chain reaction (PCR) systems and is currently only labeled for use in research purposes.
  • The frequency of inpatient falls has doubled in recent years, but it is not the result of epidural analgesia, suggests an analysis of data from the Nationwide Inpatient Sample conducted by researchers at the University of Texas Southwestern Medical School in Dallas. The data were presented at the American Society of Anesthesiologists 2014 Annual Meeting.
  • Allergic contact dermatitis (ACD) associated with implantable medical devices can promote skin cancer development as suggested in a new case report and experimental findings published online in The Journal of Clinical Investigation.
  • Gut microbes may be a key link between jet lag and obesity, suggests research done in mice and confirmed in humans. The findings could provide a mechanism for common metabolic changes seen in humans with disrupted circadian rhythms, including frequent flyers and shift works. The research is published online October 16 in Cell.

Never Hurt Ego of a Person

It is a well known Vedic fact that if you hurt ego of a person, he will never forgive you, especially, if you have insulted or implicated him in character assassination, cast implications, financial embezzlement or in public, you call a woman as old or a man as impotent.

For example, if I hurt ego of a chowkidaar, next time if a patient comes to the hospital gate and asks where Dr. KK Aggarwal is, he only has to whispery, “which Aggarwal, whose cases never survive?”

Also never hurt the ego of a person who is drunk. Under the influence of alcohol, a person loses his capacity to judge persons and situations. If you provoke him, he can be destructive and aggressive.

The Stone on the Road

One day a farmer, walking down the street in a small town came across a large stone in the middle of his path. The farmer complained: "Who could be so careless as to leave such a big stone on the road? Why does someone not remove it?" He went away complaining.

The next day, the same thing happened with a milkman. He too went away grumbling but left the stone as it was.

Then one day, a student came across the stone. Worried that someone may fall over it and hurt himself, he decided to push it aside. He pushed long and hard all by himself and eventually managed to remove the stone from the path. He came back and noticed a piece of paper where the stone was kept.

He picked the paper and opened it. Inside was written, "You are the true wealth of this nation."

There are two kinds of people
Talkers and doers.
Talkers merely talk, while doers do.

The moral of this story is that if you don't want to get involved you have no right to criticize Become the change you wish to see in this world.

  • A study of hospitalized cardiac patients has shown that effective treatment with positive airway pressure therapy reduces 30-day hospital readmission rates and emergency department visits in patients with both heart disease and sleep apnea. The study is published in the October 15 issue of the Journal of Clinical Sleep Medicine.
  • Limiting saturated fats can improve the lipid profile, suggests a study published online in the Journal of the American Heart Association. Researchers noted that bingeing on muffins loaded with saturated fats for 7 weeks raised lipid levels in healthy, normal–weight young adults, but those who consumed muffins made with polyunsaturated fat improved their plasma lipoprotein profiles.

  • New research shows that the likelihood that a child with attention-deficit/hyperactivity disorder (ADHD) will receive stimulant medication goes beyond their symptoms; low maternal education increases the odds while immigrant status decreases the odds that a child with ADHD will receive medication. The study was published in the October issue of the British Journal of Psychiatry.
  • A group-oriented early childhood intervention program that involves families, a dietician, and a psychologist can reduce the risk for childhood obesity by 84% by the age of 2, reports a new study presented recently at the American Academy of Pediatrics 2014 National Conference and Exhibition.

Bihar chief minister Jitan Ram Manjhi on Friday warned he would chop off hands of doctors found neglecting poor patients.  "I am ready to face the consequences, but cannot let doctors play with lives of the poor," he said while addressing a public meeting after inaugurating a hospital in East Champaran district.  Manjhi directed East Champaran DM to treat poor with compassion if they sell off their land given by the government. 

"Action should rather be taken against those who purchase such plots,'' he said. "Such lands are not transferable. They can be neither sold nor purchased. The sellers are illiterate, innocent people, but the purchasers are educated, clever people.''  Earlier interacting with mahadalits near Motihari, the CM asked them to maintain hygiene. "Shun alcohol and spend the money saved on sending your children to school," he told them. 

Manjhi asked them to clean their toilets themselves. "I clean my toilet myself," he said and added people call him mad as he visits dalits to know their grievances. The CM said residents of dalit localities would be provided pure drinking water and toilets so that they do not have to go for open defecation.  "The urban landless poor would be given at least five decimals of land instead of 3.5 decimals," Manjhi said and added every village of over hundred families would be linked with road and every home would have a toilet. Gaining Weight Losing Strength versus Losing Weight Gaining Strength

When we gain weight, we must acquire more strength and when we lose weight, we must lose the strength. This is a fundamental medical principle.

If we gain weight and feel week, it is a disease and when we lose weight and gain strength, we are recovering from the disease. One is not supposed to gain more than 5kg of weight after the age of 20 years. Any weight gain after that will only be due to accumulation of fat which leads to insulin resistance.

Insulin resistance does not allow food to convert into energy. In the state of insulin resistance, whatever you eat, it is converted into fat. As it is not converted into energy and you feel week. When you reduce insulin resistance by drugs or walking, metabolism becomes normal and whatever you eat gets converted into energy and you start gaining strength.

Youth Festival to Propagate Preventive Health and Hygiene Measures

New Delhi 18th October 2014– Heart Care Foundation of India, a leading national non-profit organization is committed to making India a healthier and disease free nation. On the 4th day of its annual flagship event–The 21st MTNL Perfect Health Mela organized Medico Masti festival an inter college youth fiesta at the Talkatora Stadium, New Delhi. Thousands of students of across 50 colleges, medical and non-medical from Delhi NCR participated in the event.

On the spot competitions like fashion show, choreography, rock band, etc were organized as part of the festival. Other competitions included western and classical dancing, quiz contest, singing competition, etc. Infotainment is a new concept of creating health education and in this module education is interlinked with entertainment.

Addressing the event , Padma Shri, DST National Science and Communication and Dr. BC Roy National Awardee and the President of the Heart Care Foundation of India, Dr. KK Aggarwal said “That to prevent the increase  of heart diseases in the nation, the prevention must start in college life. He said that college students should follow the principles of formula of 80 which is as under: To live more than  80 years without life style diseases keep your Abdominal Circumference, Lower Blood Pressure,  LDL (bad) Cholesterol, Pulse Rate and Fasting Sugar all lower than 80.Anyone who gains weight of more than 5 kg after the age of 18 in girls and 20 in boys is obese and overweight. Any weight gain at this age should be avoided.”

 Dr. Smita Mishra Sr. Cardiologist Fortis Hospital and Dr. O.P Yadava C.E.O. National Heart Institute added, “The youth must learn to say no to drugs, alcohol and tobacco. They also must abstain themselves from casual sex, especially if it is unprotected.”

The Mela had facilities like 3D echo cardiograph, ECG and other basic consultation will be provided. Those people can afford treatment will be guided and people who cannot afford treatment will get a subsidiary or free heart surgery. Keeping in mind the healthcare needs of the poor and needy, special free of diagnostic facilities and cardiac interventions will be provided by National Heart Institute for rheumatic heart disease and Medanta –Medicity will focus on congenital heart disease and also MTNL had set up live video conferencing unit at the mela with Medanta-Medicity.

The Perfect Health Mela is an annual mass health awareness module started in 1993, catering to people from all age groups and walks of life; the Mela incorporates activities across categories such as health check up camps, entertainment programs, lifestyle exhibitions, workshops and competitions. This year’s Mela will feature a first of its kind SAIL Heart and Health Pavilion to provide a complete solution to a person’s overall healthcare needs. People will now not only be able to learn about preventive health measures through interactive entertainment modules but will also be able to seek treatment and consultation under one roof. This year’s Mela was organized in association with NDMC, MCD, SAIL and MTNL and its theme is “Swach Baharat, Swasth Bharat”. The main philosophy behind this theme is that by improving the cleanliness of the country, several leading diseases such as dengue, typhoid, malaria, cholera can be prevented.

Medicolegal Query

Can doctor–couple dispense drugs without license?

Dr P Kamalasanan, a private medical practitioner from Kollam in Kerala was punished last month for dispensing drugs without a license. Additional Sessions Judge, Kollam on August 7 sentenced Dr Kamalasanan of one–day "imprisonment" and also fined him Rs 1.20 lakh on the charges of running a pharmacy without a license from his clinic. Appearing before the court, Dr Kamalasanan reportedly served the sentence by remaining there till the rising of the court.

View1: Under the rules, only a single–doctor clinic is exempted from the process of obtaining a license before dispensing drugs and any clinic run with the help of more than one doctor falls under the category of hospital, for which it is mandatory to obtain a license to dispense medicines.

View 2: The Drugs & Cosmetics Act, 1940 & Rules, is very clear that private clinics, the place where professional activities of registered medical practitioners take place, are exempted from the drug licence. Private clinics are fully protected from harassment of drug controllers.

Clause–5 of Schedule–K of the Drugs and Cosmetics Rules allows a single doctor to stock, prescribe or dispense medicines without having a license; it does mandate a clinic run by more than one doctor to obtain a license.

A doctor cannot dispense medicines on the prescription given by another doctor; neither can a visiting doctor dispense medicines from that clinic. Item 5. Drugs supplied by a registered medical practitioner to his own patient or any drug specified in Schedule C supplied by a registered medical practitioner at the request of another such practitioner if it is specially prepared with reference to the condition and for the use of an individual patient provided the registered medical practitioner is not (a) keeping an open shop or (b) selling across the counter or (c) engaged in the importation, manufacture, distribution or sale of drugs in India to a degree which render him liable to the provisions of Chapter IV of the Act and the rules there under."
